To obtain an evaluation report, you must first complete an application form. Please select one of the formats from the list below to download a form. You can then print the document or save it.
Application for Individuals:
- Microsoft® Word 97-2003 - Select this format if you want to download and complete the application using Microsoft® Word (or any word processor that can read Word documents).
- PDF (Portable Document Format) - Select this format if you would like to view and print the application using the Adobe® Acrobat® Reader. If you do not have the Adobe® Acrobat® Reader software installed on your computer, it is available as a free download from Adobe.
Application for Attorneys:
- Microsoft® Word 97-2003 - Select this format if you want to download and complete the application using Microsoft® Word (or any word processor that can read Word documents).
- PDF (Portable Document Format) - Select this format if you would like to view and print the application using the Adobe® Acrobat® Reader. If you do not have the Adobe® Acrobat® Reader software installed on your computer, it is available as a free download from Adobe.
Application for Teacher Certification for the States of Illinois and New York ONLY:
